We are seeking a nurse practitioner (NP) or physician assistant (PA) for primary care rounding in a behavioral health facility in the Holyoke, MA area. This is a full-time role with no on-call requirements.

• 4/5 Days per week
• 8am - 4pm
• No Call
• Some weekend and holiday coverage required

Responsibilities:

• Perform primary care in a specialty setting

• Manage chronic disease, diagnosis illness and medical conditions, and implement plan of care

• Order and interpret lab results

• Prescribe and monitor medications

• Consult with physicians and coordinate patient care with other healthcare practitioners

• Educate patients and their caregivers on coping skills

• Build relationships with your patients, patients family members, and facility staff to facilitate effective treatment delivery

• Approximately 15 consultations per day

Requirements:

• Primary care experience required

• New grads encouraged to apply

• Active and unrestricted MA NP or PA license or application in process

• Active and unrestricted DEA or application in process

• Excellent communication skills
