Saint Joseph Hospital in Elgin, Illinois is seeking a dedicated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Primary Care team. This full-time role offers the opportunity to deliver comprehensive, patient-centered care in an established outpatient family medicine setting. Position Summary & Qualifications
- Full-time position in a busy outpatient Primary Care clinic
- Provide full-spectrum care including preventive health, chronic disease management, and acute care visits
- Collaborate with physicians, other APPs, and clinical staff for coordinated, team-based care
- Maintain timely documentation in the electronic medical record (EMR) system
- Schedule: Monday-Friday, with minimal call responsibilities
- Must be a licensed NP or PA in the state of Illinois
- Board certified in Family Medicine (NP) or possess the appropriate PA certification
- Minimum 12 years primary care experience preferred
- Strong clinical decision-making, interpersonal, and documentation skills

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $125,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
