Our ideal candidate will focus on optimizing employees’ and their dependents’ overall health by assessing their current health risks and providing disease prevention, education and detection.

Licensure / Certification / Registration:

• Physician MD/DO credentialed from the Indiana Medical Licensing Board obtained prior to hire date or job transfer date required.
