Job Description

American Traveler is seeking a Physical Therapist with at least one year of home health experience and dual state licensure in Vermont and New Hampshire.

Responsibilities

• Positions available for day shifts in a home health setting
• Covers areas including White River Junction and Bradford in VT, and Lebanon in NH
• Average patient visit length is 45 minutes allowing for meaningful therapy sessions
• Work as part of a cohesive interdisciplinary care team
• Responsibilities include screening, evaluating, re-evaluating, and treating patients in their homes
• Physical Therapist will develop and implement treatment plans and document evaluations, goals, and objectives
• Coordinate care and communicate effectively with the broader healthcare team
• Educate patients and families about treatment plans and therapy rationale
• Environment supports independence with strong team collaboration

Requirements

• BS in Physical Therapy required
• Current Physical Therapist license in VT and NH, or valid compact license
• Minimum one year of experience as a PT in a home healthcare setting
• BLS/BCLS certification required
• License required if applicable
