As a Family Practice/Primary Care Nurse Practitioner, you perform comprehensive in-home health risk assessments to identify physical, mental, and social health needs of patients. You obtain vital signs, measure BMI, review medications, conduct physical exams, and provide education and counseling to patients and their families. This part-time role offers flexible hours, fixed payment per screening, malpractice coverage, and requires active state licensure and board certification.

Perform Health Risk Assessments (HRAs)on patients in theirhomes
Nurse Practitioners with HRA experience preferred
Nurse Practitioners conduct comprehensive in-home health riskassessment to identify all active and chronic disease conditions as well asdetermine all physical, mental and social needs present at the time of thevisit
Nurse Practitioners will obtain vital signs, measure BMI, reviewpharmacological therapy and conduct a physical examination
Nurse Practitioners educate and counsel patient and family onany conditions identified during assessment and screening procedures.
Part time and Flexible: Set your own hours, must be availablefor at least 8-10 weekly hours (visits can be scheduled 7 days a week)
Fixed Payment at $130 per in-home screening; mileage and no show reimbursement
Each screening takes 1hr. on average and there is no prescribing
Malpractice Coverage, paid training, scheduling and EMR systemprovided
Requirements:
- Must have active Nurse Practitioner state license
- Must have active Nurse Practitioner Board Certification
- Must have valid state driver license and automobile insurance
- Knowledge of clinical standards of care
- Ability to complete and clear a background and credentialing screen
- COVID vaccination NOT required
