A Board-Certified Hospice/Palliative Medicine Physician will provide patient-centered symptom management and advance care planning through a hybrid model combining telehealth and in-home visits. An innovative, national healthcare organization is seeking a Board-Certified Palliative Care Physician to join a forward-thinking, value-based care model focused on improving quality of life for patients with chronic and advanced illnesses.
This position offers a unique opportunity to practice within an integrated, multidisciplinary team that delivers in-home and virtual palliative care designed to enhance patient outcomes, reduce hospitalizations, and improve overall patient and family experience.
Position HighlightsFull-time, Monday Friday schedule
No nights, weekends, or call
Hybrid model: 60% remote (telehealth), 40% in-person home visits
Clinical position with a focus on patient care and quality outcomes
Variable daily volume (approximately 3 7 patients/day via telehealth)

Identify and manage patients appropriate for palliative care services, building and maintaining a dedicated patient panel.
Provide symptom management for pain, depression, anxiety, insomnia, and other conditions impacting quality of life.
Conduct goals-of-care and family meetings; document wishes in advance directive and POLST forms.
Support hospice transitions where clinically appropriate.
Collaborate with interdisciplinary teams, including nurses, social workers, chaplains, and specialists.
Foster strong relationships with local primary care providers, nephrologists, and community partners.
Participate in performance reviews and quality initiatives to meet both clinical and financial KPIs.
Educate and mentor care teams on palliative care principles and best practices.
Doctorate Degree (MD or DO) from an accredited medical school
Board Certification in Hospice and Palliative Medicine
Current, unrestricted medical license (multi-state licensure preferred or willingness to obtain)
Strong background in symptom management, advance care planning, and patient-centered communication
Experience with Medicare Advantage, managed Medicaid, and population health models preferred
Familiarity with integrating acute, behavioral, and social care needs
Reliable transportation required
