The Ambulatory Clinical Pharmacist provides medication therapy management, develops drug protocols, and collaborates with healthcare teams to optimize patient outcomes within ambulatory care settings. Responsibilities include patient counseling, managing collaborative practice agreements, coordinating with billing and scheduling teams, and supporting pharmacy benefit programs. The role also involves precepting residents, participating in quality and compliance initiatives, and promoting specialty pharmacy services.
The Staff Pad has partnered with a healthcare organization in Helena, MT to hire an Ambulatory Clinical Pharmacist.
Responsibilities Include:
Develop and manage drug protocols and collaborative practice agreements.
Provide medication therapy management, drug interaction review, side-effect management, and patient counseling.
Document patient care activities and monitor therapy according to clinical protocols.
Coordinate with scheduling and billing teams to enhance productivity and reimbursement.
Work with pharmacy staff on benefits investigations and assistance programs.
Collaborate with care teams to improve patient outcomes and report quality measures.
Deliver education to patients, providers, and staff.
Promote ambulatory and specialty pharmacy services across the organization.
Serve on clinical and operational committees and ensure compliance with regulations.
Precept pharmacy residents and students.
Oversee outpatient antimicrobial stewardship efforts as applicable.
Perform additional pharmacist duties as needed.
Qualifications:
Strong knowledge of drug therapy, chronic disease management, therapeutic drug monitoring, and pharmacy best practices.
Skilled in analyzing clinical data, making recommendations, and following through on interventions.
Proficient with pharmacy technology and clinical documentation systems.
Education & Experience:
Bachelor’s in Pharmacy or PharmD from an ACPE-accredited program required.
Completion of an ASHP-accredited residency and/or ambulatory care experience preferred.
Licensure/Certifications:
Licensed pharmacist in Montana required.
Board certification or specialty certification required when eligible.
Must obtain clinical pharmacist practitioner credentialing when eligible (if supporting Primary Care).
Must meet and maintain provider credentialing requirements.
