Setting: Home Health
We are seeking a licensed Occupational Therapist (OT) for a home health contract assignment. The Occupational Therapist’s primary role is to restore and maximize patient function, promote independence in activities of daily living (ADLs), and prevent further functional decline following illness or disability. This position serves patients in assigned community territories and works collaboratively with interdisciplinary care teams.
Service AreaCoverage includes assigned territories and neighboring communities as needed.
Key Responsibilities- Evaluate patients and develop individualized occupational therapy treatment plans
- Implement medically prescribed OT programs to improve functional performance
- Facilitate independence with ADLs and adaptive strategies
- Educate patients and caregivers on therapeutic exercises, safety, and equipment use
- Monitor patient progress and adjust treatment plans as appropriate
- Maintain accurate, timely documentation in accordance with home health standards
- Collaborate with physicians and interdisciplinary team members to support patient outcomes
- Perform additional duties consistent with skills, training, and scope of practice
- Active state Occupational Therapist (OT) license (must be current)
- Minimum 1 year of Occupational Therapy experience
- Home health experience preferred
- Strong time-management, communication, and clinical assessment skills
- Ability to work independently in a community-based setting
- BLS/BCLS required
- Homecare Homebase (HCHB) electronic medical record
- Bilingual skills not required
