Compensation: Base Salary $215K $245K + Quality Incentive Bonus

We re seeking a dedicated Primary Care Physician to join our growing network of neighborhood clinics delivering high-quality, value-based care to Medicare patients. Our model emphasizes outcomes over volume empowering physicians to focus on preventive medicine, patient relationships, and long-term wellness. Supported by robust care teams and advanced technology, our providers can deliver the kind of care that makes a real difference.

• Care Team: Scribe, Medical Assistant, and Social Worker support

• Onsite Resources: Lab with phlebotomist; RubiconMD specialist consult access

• EMR Systems: Canopy and Greenway

• Model: Value-Based Health Care

• Clinic Team: 3 5 providers per site (mix of MDs and NPs)

• Community Engagement: On-site wellness programs exercise, yoga, dance, and social events

• Patient Services: Complimentary transportation within 5 miles of the clinic

• Physician Partnership Track

• 6 Weeks PTO (includes holidays and CME)

• $5,000 CME Allowance
