- Administering medication to patients.
- Creating reports to track the patients' progress.
- Working with other professionals to create treatment plans.
- Cleaning and re-dressing wounds if needed.
- Measuring vital signs.
- Monitoring changes in behavior and progress.
- Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations.
- Develop and implement individualized Plans of Care (POC) in collaboration with the physician and interdisciplinary team.
- Provide skilled nursing services, such as wound care, IV therapy, medication management, catheter care, and chronic disease monitoring.
- Educate patients and caregivers on disease management, medication regimens, and home safety to promote independence.
- Accurately document all clinical visits and communications in HCHB EMR systems in a timely manner.
- Communicate changes in patient condition promptly to physicians and obtain/implement new orders as necessary.
- Collaborate with therapists, social workers, home health aides, and other care providers to ensure coordinated care.
- Maintain compliance with Medicare, state, and agency regulations, infection control policies, and safety standards.
- Participate in case conferences, quality assurance programs (QAPI), and continuing education activities.
Must have CA State RN license Must have BLS AHA Must have Home Health experience with HCHB EMR.
