Physical Therapist (PT) – Skilled Nursing Facility | Hamden, CT
MedAdventures is seeking an experienced Physical Therapist to join a skilled nursing facility in Hamden, CT. In this role, you’ll help residents regain mobility, strength, and independence while working within a supportive, collaborative rehab team.

Your Responsibilities:
- Provide individualized physical therapy services to SNF residents
- Develop and implement treatment plans to improve functional mobility
- Collaborate with PTAs, OTs, nurses, and interdisciplinary team members
- Maintain accurate and timely documentation
Qualifications:
- Active Connecticut PT license
- SNF or geriatric rehab experience preferred
- Strong clinical reasoning, communication, and teamwork skills
