Mix of site-based and in-home care
Collaborative interdisciplinary team (IDT) environment
Serve a frail and elderly population with comprehensive support
Evaluate participants and develop individualized occupational therapy treatment plans
Provide hands-on therapy in clinic settings and participant homes
Collaborate closely with providers, IDT members, and specialists
Document evaluations, progress notes, discharge summaries, and care plans in the EMR
Recommend adaptive equipment and support safe discharges
Participate in care plan meetings and performance improvement initiatives
Provide clinical guidance to COTAs and support OT students as needed
Maintain compliance with all clinical, safety, and regulatory standards
Required Qualifications
Bachelor’s degree from an accredited Occupational Therapy program
Active New York State Occupational Therapist license
Ability to travel locally within Brooklyn to participant homes and sites
Strong communication, documentation, and clinical judgment skills
Preferred Experience
At least 1 year of OT experience, ideally with geriatric or frail populations
Familiarity with community-based care or PACE-style programs
Experience using EMRs (Care Compass a plus)
Bilingual skills are a plus, depending on participant population
Ability to lift up to 50 lbs and assist participants safely
Comfortable with standing, walking, bending, and in-home visits
Up-to-date immunizations and medical clearance required
Commitment to ethical practice and ongoing professional development
Services provided at program sites and participant homes within the borough.
