Responsibilities:
- Collaborate with physicians and healthcare professionals to deliver comprehensive primary care services.
- Conduct physical examinations, health assessments, and preventive screenings appropriate to patient age and history.
- Develop and implement treatment plans, including prescribing medications and interventions.
- Order, interpret, and review diagnostic tests and laboratory results within established protocols.
- Facilitate patient education and coordinate referrals to specialists and community resources.
- Participate in hospital rounds for new consults and follow-up care, including discharge planning.
- Maintain accurate, timely documentation within the EMR system and ensure proper coding of patient visits.

Requirements:
- Graduate of an accredited Nurse Practitioner Program and licensed in New York State.
- 2-5 years of related clinical experience or an equivalent combination of education and experience.
- Demonstrated proficiency in advanced health assessment and decision-making.
- Strong organizational and communication skills, with the ability to work effectively under stress.
