WCS Healthcare is currently seeking an Case Management Nurse for a contract assignment at a Prestigious Hospital in Poughkeepsie, NY.
Job Function:
- Day Shift.
- 8am to 4:30pm.
- Assess new patients.
- Update and revise patient health care plans as needed.
- Educate patients and families about health care plans and options.
- Make clinical decisions for routine patient care.
- Accurately document and submit medical records.
Job Requirements:
- NYS RN license.
- NYS PRI Certification.
- 1-2 Years Acute Care Discharge Planning Experience.
