Home Based Primary Care Physician Role Overview:
The Home Based Primary Care (HBPC) Physician and Patient Care Manager (PCM) will work under general supervision to provide primary care to veteran beneficiaries. Duties include examination, diagnosis, prevention, and treatment of various medical conditions, as well as overseeing non-credentialed healthcare providers. The role requires Board Certification or Board Eligibility in Family Practice and BLS certification.
Key Responsibilities
Primary Care Services:
- Serve as a PCM for the designated patient population.
- Conduct examinations to determine the presence and extent of diseases, injuries, or conditions.
- Order and interpret diagnostic procedures such as laboratory tests and x-rays.
- Make diagnoses and prescribe appropriate treatment plans.
Clinical Oversight:
- Provide clinical oversight of non-credentialed healthcare providers.
- Serve as a physician supervisor for Nurse Practitioners (NPs) or Physician Assistants (PAs).
On-Call Duties:
- Provide after-hours and weekend telephone on-call from home as scheduled.
Procedural Responsibilities:
- Perform procedures including reduction of simple fractures, emergency delivery, joint aspiration/injection, ECG performance and initial interpretation, basic radiograph interpretation, regional anesthesia, splinting/casting/immobilization of simple fractures, crisis intervention, and Basic Life Support (BLS).
Administrative Duties:
- Use automated systems (e.g., VISTA) for entering clinical orders and prescriptions.
- Complete workload accountability documents (e.g., CPRS) weekly.
- Participate in clinic performance improvement and risk management activities, including peer reviews and chart reviews.
- Attend mandatory training as required.
Team Collaboration:
- Work collaboratively with other physicians, nurse practitioners, registered nurses, and support staff.
