Position SummaryA well-established medical practice is seeking a full-time Primary Care / Family Medicine Physicians to join a collaborative care team in a community-based outpatient setting. - Provide comprehensive primary care services across the lifespan
- Coordinate patient care, including referrals, follow-up, and care transitions

- Maintain accurate and timely clinical documentation that meets regulatory and payer requirements
- Participate in inpatient and outpatient consultation and call coverage as needed
- Engage in academic, educational, or research activities as requested
- Board Certified or Board Eligible in Family Medicine or Primary Care specialty
- Active state medical license or eligibility to obtain licensure
- Successful completion of an ACGME-accredited residency program
